# Surgical Scar Revision Cost in India: What Sets the Price

Surgical scar revision cost is not one number, which is structural, not evasive. A two-centimetre scar excised under local anaesthesia and a burn contracture released with a skin graft under general anaesthesia are both scar revision surgery, yet sit at opposite ends of the range.

Three things set most of the price: which technique the scar needs, how many scars are treated in one sitting and whether the scar is keloid-prone. The first two can be worked out in front of a mirror.

The third catches people out: for a keloid-prone scar, the injection course is usually the largest budget line, not the operation.

## Which scar surgery does your scar actually need?

Scar surgery is not one operation but three: excision with layered closure, Z-plasty or W-plasty, and contracture release with a graft or flap. Each tier sets its own floor under the price, because operating time and anaesthesia scale across them.

Timing sits ahead of price, though. Except where a scar restricts movement or keeps breaking down, elective revision is usually deferred until the scar has matured, commonly 6 to 12 months and longer on high-tension sites or while the scar is still red and firm, because a settling scar often improves on its own and revising too early gives a poorer result.

**Tier one — excision with layered closure.** The scar is cut out, the edges undermined to meet without tension, then closed in layers. Single scars of a few centimetres are done under local injection in an outpatient room in 30 to 60 minutes, same-day discharge, no anaesthetist — the cheapest tier.

**Tier two — Z-plasty and W-plasty.** Both re-cut the scar into interlocking limbs. Z-plasty gains length, which releases a scar pulling across a joint; W-plasty gains none and only breaks the straight line. More operating time, higher fee.

**Tier three — contracture release with graft or flap.** A released post-burn contracture leaves a defect too large to close directly, so it is resurfaced: general anaesthesia, theatre and anaesthetist charges, a donor site, sometimes overnight admission, then splinting. Several times the tier-one figure.

## The keloid trap: the course costs more than the operation

For a keloid-prone scar, the excision is not what decides the budget. Recurrence after excision alone is high: [published series report 45% to 100%](https://www.ncbi.nlm.nih.gov/books/NBK507899/), varying by site, technique and length of follow-up. The returning keloid is often larger, so surgery is not offered here without an adjuvant plan. Steroid injection is the adjuvant used most often. For repeatedly recurring or high-tension keloids there are reserved options: intralesional 5-fluorouracil, used off-label, supported by smaller studies than steroid and carrying its own risk of injection-site pain, ulceration and pigment change; and post-operative superficial radiotherapy, which is delivered by a radiation oncology unit rather than at this clinic and carries its own risks. Either is discussed at examination, with referral arranged where it is not offered here. Sittings are billed separately at ₹5,000 to ₹8,000 each, four to six weeks apart, commonly three to six: roughly ₹15,000 to ₹48,000 outside the operative quote.

An adjuvant course brings reported recurrence down: across published series of excision plus a steroid course, most report under 50%, and some considerably lower. Those are literature ranges rather than promises, and they vary widely by site, size, skin type and technique. Low-tension sites cost less to control. Intralesional steroid is not side-effect free. Skin at the injection site can thin and surrounding skin lighten, more noticeably in darker skin tones, and small vessels may become visible. The injections are uncomfortable; dose and interval are adjusted each sitting.

## What does scar removal treatment cost per session or per procedure?

One thing to settle first: no operation removes a scar. Revision replaces it with a new one planned to be flatter and better aligned with skin lines — improvement, not erasure, is what is priced.

Two billing units sit inside one consultation, which makes quotes look inconsistent. Surgery is a single procedure fee covering surgeon, anaesthesia, consumables, dressings and planned follow-ups. Non-surgical therapy is charged per session, so the number that matters is the session price times a realistic count — frequently more than a small excision.

## What else moves surgery scar removal cost

Scar count is where a patient can genuinely move the figure: several scars treated in one sitting cost less per scar than the same scars across separate visits, because setup and theatre time are paid once. Site matters alongside it — facial scars demand more exacting closure, a scar crossing a joint needs a tension-releasing technique. ## Is scar revision covered by health insurance in India?

It depends on why the scar is being revised. India's standard cosmetic-surgery exclusion (IRDAI Excl08) does not apply to reconstruction after an accident, burns or cancer, nor to treatment needed to remove a direct and immediate health risk — so a post-trauma case is worth putting to the insurer.
