# Choosing Between Gynecomastia Surgeons: A Consult Checklist

When the chest still looks puffy around the nipple after months of gym work, the real question is not "what is gynecomastia" but "who should I trust to fix it." Pages for gynecomastia surgeons across India tend to answer with a price by grade and front-on before-and-after photos. Neither tells you whether the operation on offer removes the tissue actually causing the shape.

That matters because gynecomastia is usually a mix of firm breast gland and softer fat, and the two are treated differently. Liposuction removes fat well but tends to handle dense gland poorly. A plan built only around liposuction can leave the gland under the nipple in place, one of the commoner reasons men return with a chest that still looks puffy.

This guide sets out what a proper consultation should examine, where lipo-only plans can fall short, what recovery and risk look like, and the questions that help you vet any surgeon, including us.

## Who this article is for

This guide is written for men comparing gynecomastia doctors near them in Gurgaon or Delhi NCR who see the same claims on every clinic page, and for anyone quoted a fixed "lipo package" by grade before an examination.

It also helps men unsure whether their chest is gland, fat or both (the [gynecomastia grades guide](/blog/gynecomastia-grades-treatment-options-guide/) explains grading), parents of teenagers with breast enlargement wondering when surgery makes sense, and anyone considering [gynecomastia surgery in Gurgaon](/procedures/gynecomastia/) who wants to choose carefully.

## What gynecomastia is, and why the type decides the operation

Gynecomastia is enlargement of glandular breast tissue in men, usually linked to an imbalance between oestrogen and testosterone activity. Pseudogynecomastia is chest fullness from fat alone, without increased gland. Most adult chests that reach surgery have some of both, and the proportion of gland to fat largely decides whether liposuction, gland excision or a combination is suited.

### True gynecomastia vs pseudogynecomastia

True gynecomastia typically feels like a firm, rubbery disc directly beneath the nipple and areola, and it can be tender when new. Pseudogynecomastia feels soft, spreads evenly across the chest and often reduces with weight loss. Established gland does not reliably shrink with diet or exercise, which is why men who have lost weight can still have a projecting nipple. Recent-onset enlargement can sometimes settle once its cause is treated.

### Common causes

- **Puberty:** hormonal shifts commonly cause breast tissue in teenage boys, and in many it settles on its own within months to a couple of years
- **Medicines and substances:** anabolic steroids, some prostate, heart and blood pressure medicines, certain antacids and psychiatric drugs, and possibly heavy alcohol or cannabis use
- **Medical conditions:** liver or kidney disease, an overactive thyroid, low testosterone and, rarely, hormone-producing tumours
- **Weight gain,** which adds fat and can increase oestrogen activity
- **No identifiable cause,** which is common in adults

## Why can lipo-only gynecomastia surgery leave the gland behind?

Liposuction cannulas pass easily through fat but tend to slide around firm, fibrous gland rather than through it. When the gland sits as a disc under the areola, suction alone often removes the surrounding fat and leaves that disc in place. The chest looks flatter at the edges while the nipple still projects, the persistent puffy nipple behind many revision consultations.

This does not make liposuction wrong. For pseudogynecomastia, or a mostly fatty chest, liposuction alone can be the right operation. The problem is a technique chosen before anyone has examined the chest, which is exactly what a gynecomastia "package" priced by grade over the phone assumes. Where a firm gland is present, it is usually excised through a small incision along the lower edge of the areola, with liposuction used to blend the surrounding chest so the contour looks even.

## What should a gynecomastia doctor examine at the first consult?

A proper consult establishes four things before recommending surgery: the grade of enlargement, how much is gland versus fat, whether there is loose or excess skin, and whether a medical cause needs ruling out. If a consultation skips the examination and moves straight to a price, the plan is a package, not a diagnosis.

### Grade and skin excess

Grading runs from small enlargement with no extra skin to large enlargement with drooping skin. Higher grades with loose skin may need [skin removal and longer scars](/blog/male-chest-reduction-surgery-gland-fat-skin/), which liposuction alone cannot address.

### Gland versus fat

Palpation, feeling the tissue behind the areola between finger and thumb, is usually enough to detect a firm gland. Where the picture is unclear, an ultrasound can help show the proportion of gland to fat. A one-sided, hard or fixed lump, nipple discharge or skin changes call for imaging and a specific workup, because men can, rarely, develop breast cancer.

### Endocrine screen

Adolescents, and adults with new, painful or rapidly growing enlargement, should have a medication and lifestyle history and, where indicated, blood tests such as testosterone, oestradiol, LH, hCG, prolactin, thyroid, liver and kidney function, along with a testicular examination. Treating a reversible cause can sometimes avoid surgery altogether.

### Who is a candidate

Suitable candidates tend to be men with stable enlargement, a steady weight, no anabolic steroid use, reasonable general health and realistic expectations. Teenagers are usually observed first, with surgery generally considered once development has settled or when enlargement is severe and persistent.

## Recovery, compression and the risks to discuss

Most men return to desk work within one to two weeks of gynecomastia surgery and wear a compression garment for around four to six weeks. The final contour typically shows at three to six months. Risks include seroma, bleeding, asymmetry, sensation change and scarring, which deserve a specific conversation, not just a line on a consent form.

### Recovery timeline

- **First week:** soreness and swelling, often with some bruising, managed with prescribed pain relief; drains, if used, usually come out within a few days
- **Weeks 1-2:** most men resume desk work and light walking
- **Weeks 4-6:** the compression garment continues; chest workouts and heavy lifting are typically reintroduced after this
- **Months 3-6:** residual swelling and firmness settle; scars keep fading for up to a year

### Risks

- Seroma (a fluid collection), which may need draining
- Haematoma (bleeding under the skin)
- Asymmetry between the two sides
- Change in nipple sensation, usually temporary but occasionally lasting
- Visible or raised scarring
- Infection and, rarely, compromised blood supply to the nipple
- Contour irregularity or residual fullness, and the general risks of anaesthesia

### Contour deformity and revision risk

Two opposite errors are among the commonest reasons for revision. Over-resection, removing too much tissue under the nipple, can leave a saucer or crater deformity in which the nipple sinks or tethers to the muscle. Under-resection leaves residual gland and the original puffy look. Revision is generally harder than the first operation because scar tissue distorts the tissue planes, which is why the [guide to second opinions after gynecomastia surgery](/blog/gynecomastia-revision-surgery-second-opinion-guide/) advises allowing full healing before reassessing.

## Questions to ask gynecomastia surgeons before you book

The questions that separate a gland-excision surgeon from a lipo package start with the examination and end with the surgeon's own revision record. A surgeon who plans properly can tell you what they found in your chest, why they chose the technique, how they avoid over- and under-resection, and how often their patients need revision.

1. **What is your qualification and registration?** In India, the equivalent of a board-certified plastic surgeon holds an MCh or DNB in Plastic Surgery and is registered with a state medical council. The [liposuction surgeon vetting checklist](/blog/choosing-liposuction-surgeon-gurgaon/) explains how to verify both.
2. Did you find gland, fat or both, and roughly how much of each?
3. Will the gland be excised, and through what incision?
4. Is there skin excess, and how will it be managed?
5. Do I need blood tests or an ultrasound first?
6. How do you avoid a saucer deformity?
7. How often do your gynecomastia patients need a revision, and for what reason most often?
8. **Can I see before-and-after photos with side and oblique views, for a similar grade?** Front-on photos can hide projection under the nipple that a lateral view shows.
9. Where is the surgery done, who gives the anaesthesia, and whom do I call after hours?

## Choosing a gynecomastia surgeon in Gurgaon

Many clinic pages in Gurgaon and Delhi NCR lead with a locality and a grade-based price, and neither tells you much about the plan for your chest. What helps is a consultation that includes an examination, an explanation of gland versus fat in your chest, and a quote given after that assessment. An itemised quote covering surgeon, anaesthesia, theatre, garment and follow-up is easier to compare than a single package figure, and more useful than searching for a gynecomastia hospital near me.

In our practice in Sushant Lok 1, we typically start a gynecomastia consultation with a history and examination, followed by a technique plan and a written estimate. Consultations are with Dr. Shikha Bansal, MBBS (Gold Medalist), MS General Surgery, MCh Plastic & Reconstructive Surgery, Haryana Medical Council Reg. No. 24859. ## Frequently asked questions

### Is gynecomastia surgery painful?

Most men describe soreness and tightness rather than sharp pain, worst in the first few days. It is usually managed with prescribed oral pain relief, and the compression garment tends to ease movement.

### Are the results of gynecomastia surgery permanent?

Gland tissue that has been removed does not regrow, so results are usually long-lasting, though any gland left behind can enlarge again with steroids, certain medicines or a hormonal cause. Remaining fat cells can still enlarge with significant weight gain, which can change the contour.

### Can gynecomastia come back after surgery?

True recurrence is uncommon when the gland has been adequately removed, but it can happen, particularly with anabolic steroids, certain medicines or an untreated hormonal cause. Fullness persisting after a lipo-only operation is more often residual gland than recurrence.

### Will gynecomastia surgery leave scars?

Gland excision usually leaves a small scar along the lower edge of the areola, where the colour change helps disguise it, plus tiny liposuction entry points. Higher grades needing skin removal involve longer scars.

### How do I find the best gynecomastia surgeon?

Start with credentials: an MCh or DNB in Plastic Surgery and a valid medical council registration. Then judge the consultation itself, whether your chest was examined and gland and fat explained, and ask for lateral-view photos of similar cases.
